Describe the symptoms and the timeline of the reaction.

I am requesting Reconsideration at the Request for Benefits (RFB) from the countermeasure Injury Compensation Program for my husband falling and banging his head and shoulder against the hard floors and walls, damaging his brain and other trauma without being aware this would happen. I plead that you do the right thing by compensating me for Mr. Harris injuries and death. I am requesting Reconsideration of my letter based on Mr. medical conditions with him becoming paralyzed and tremors along with other medical conditions. He became bedridden, not able to walk or move body, not able to talk. He died from injuries and health complications January 23, 2024. That’s a day I want forget.
Spouse went to the emergency room frequent and always had two or three infections and other conditions. He was given antibiotic in IV fluids. He never went to the hospital so regular until after he got Covid-19 vaccine. The Covid-19 vaccine contributive to and cause serious adverse actions. Inpatient hospitalization or prolongation of existing hospitalization incapacitated or disruption his ability to conduct a normal life. Important medical events resulted in death, life-threatening, required hospitalizations. He became paralyze, tremors, hit his head and right shoulder on floor and walls, chest and other body parts on the hard floors, could no longer eat or drink fluids.

He could no longer move lower body and arms, fell on the floor hitting his head burst his left ear on the night stand, landing on right shoulder caused pain, tremors kept happening, headaches, lot of nose bleeds, laid down majority of time, loss appetite, food taste bad, malnourish, slacked up on appointments. 11-09-2022, he had lot of problems after that last shot. About a week later he started feeling bad, nausea and vomit a lot, loss appetite, and laying down. Mr. Harris had never experienced tremors or paralysis before. He had a few tremors. He had it in right arm, waist down with falls through out days. I hope that you make this right for him and me. The ER doctor admitted him in Saint Francis Hospital from November 27, 2023 to 01-04-2024: for high blood pressure and oxygen level dropped upon standing he fell.
I am applying for Countermeasure Injury Compensation Program (CICP) for spouse permanent paralyzed and tremors fall. He laid down most time, loss appetite, malnourished, and slacked up on appointments. From November 2023 to January 2024, he was fatigue. William loss weight from 295 pounds to about 155 pounds from 2022 -2024, socially isolated, could not swallow, bedridden, headaches, difficulty breathing, chills, chest pain, SOS with tightness in chest (heavy weight), frequent hospitalizations for left lung collapsed 5 or 6 times surgeries, shoulder and muscle pain, insomnia, loss of taste and smell, loss of livelihood, oxygen and high blood pressure upon standing, weakness, bedridden, socially isolated, Tylenol for headaches, health declined, and death on January 23, 2024. He developed eating problems and always had a good appetite.

spouse told me that I did not know how bad he felt. He also had right foot pain, joint and chest pain. He lost a lot of weight from November 13, 2022 -January 23, 2024. With paralysis and tremors, he fell frequently and did not have control of his body. With no quality of life or being around family members. He threw up for three days, and did not drink fluid or eat because he could not keep it down.

11-09-2022-11-13-2022: He was not having so many medical conditions until four days after consuming COVID-19 vaccine he started laying down not feeling well. Colitis/diverticulitis cause his side and stomach pain, feeling bad, decreased appetite, difficult swallowing food, beverages, and viral infection; chronic hypoxemic respiratory failure, it could have caused his lung to collapse many times. It caused SOB, trouble breathing, fast heart rate, low oxygen, chest pain, insomnia, falls and tremors, paralysis/paralyzed, back and arm pain by falling face downward on hard floor and wall (Tylenol) shoulder pain, weakness, transfusion; sort throat, high blood pressure, frequent nosebleeds. He had been in the hospital a lot, days and months at a time. Being paralyzed and tremors, all those hard falls with head and shoulder hitting floor messed up his head, shoulder, back and chest with trauma. I was his caregiver. I observed everything that happen to him. He had pain, soreness, fatigue, muscle pain, joint pain, nausea/vomiting, shortness of breath. To determine eligibility, generally I submitted a Request for Benefits Package to the CICP within one year.

11-09-2022 - 01-24-2024: he did not have all these medical problems until consuming COVID-19 vaccine last shot. Colitis/diverticulitis caused his side and stomach pain, difficult swallowing food and pills, beverages, and viral infection; it could have caused his lung to collapse many times. It caused SOB, chest pain, fast heart rate, low oxygen level, insomnia, back pain by falling face downward on hard floors, and subsequent death. I was his caregiver. I observed everything that happen to him.
01-04-2024 - 01/24-2024: spouse was bedridden. He was paralyzed, could not move his body. He could not hardly eat he could not move hands. He got where he stopped talking and whisper. I called the nurse at a skilled nursing facility for rehabilitation. I told her that Mr. Harris was deteriorating. The following day the nurse called me back, she told me he was deteriorated. I told family members that needed to see him go because he only had a short time left before he was gone. he was administered or used the covered Countermeasure Injury Compensation Program died because of his injuries and conditions. There are reports of headaches, chills, frequent infections, lung collapsed, paralysis in the submitted medical records. The submitted medical records indicated that Mr. Harris’ cause of death was illness and injuries. His voice became weak, he whispered low. Mr. Harris died on January 23, 2024, because of his injuries resulting from the use of a COVID-19 vaccine. He was supposed to came home on 01-24, 2024. I had to call HOSPICE to take care of him. He had pain, soreness, fatigue, headache, muscle pain, joint pain, chills, SOB. He suffered with respiratory problems, dizziness, exhaustion, muscle and joint pain, debilitating shoulder injuries from falls. These serious injuries, paralysis and tremors contributive to my husband death. It killed him slammed his head against hard floor and wall; death was caused by administration of COVID-19 vaccine. It caused significant loss of function and hospitalizations. Saint Francis Hospital and his right foot was hurting before being discharged from the hospital.

01-04-2024 - 01/23-2024: He became bedridden. He was paralyzed and could not move his body. He barely could not eat or move hands to eat. He got where he stopped talking and whispered. I called the nurse at a skilled nursing facility for rehabilitation. I told her that Mr. Harris was deteriorating. The following day the nurse called me back, she told me he had deteriorated. I told family members that needed to see him to go before it was too late. He only had a short time before he was gone. The submitted medical records indicated that Mr. Harris’ cause of death was illness and injuries. Spouse died on January 23, 2024 about 11:30 pm, because of his injuries resulting from the use of a COVID-19 vaccine. He stopped walking or talking, bedridden, he was supposed to came home on 01-24, 2024, but he died.
2021: He had 2 to 3 infections every time he went to the hospital for emergency treatments caused by COVID-19 vaccine. He had chest pain, SOB very hard, he barely walked through the house needing to sit down and rest. When I returned, he had surgery procedure with chest tube inserted. His left lung had collapsed. Harris was admitted to ICU for emergency treatment of lung. I told the doctor he had COVID-19 vaccine. It caused his left lung to collapse. Physician told me that he could say a lot about COVID-19 vaccine. Lot of hospitalizations and other body parts brought on by COVID-19 vaccine. • 03-23-2021 – 07-27-2022: Mr. H present ED for visits with his primary care physician (PCP) for follow-up of his medical conditions and acute issues such as spasms of the back, physical deconditioning, weakness in arms and legs, high blood pressure, ankle and legs edema. He was also seen in the emergency department (ED) for nausea, vomiting, and shoulder pain that radiated down his arm to his hand. He was given referrals to an orthopedist and cardiologist. When he hit the floor, he always landed on his right shoulder and head. He received home Physical Therapy.
11/09/2022: about 4 or 5 days after he got the last Booster vaccine, Mr. Harris almost became bed ridden, stopped eating, and vomiting. He slid off the bed to the floor because he was paralyzed. His head hit the night stand and burst left ear; he bled a lot. I finally knew as I observed him slide off the bed, his body was paralyzed. He could not move arms to catch walker or pull himself up off the floor. I begged him to eat and rushed to grab him to prevent hitting his head on the floor, walls, bathroom and falling on hot stove. He always had 2 or 3 infections every time he went to the hospital for emergency treatments. After COVID-19 vaccine, he had never had so many problems and complications before.
• Numbness in extremities, muscle weakness in legs and arms, difficult breathing, blood pressure fluctuations, severe headaches from hitting head and shoulder on the floor and wall, hospitalization and intensive care unit, viral infections (respiratory), bacterial infections, muscle weakness, complication speaking.
• 11/13/2022: He barely consumed anything, he stayed in bed feeling bad. The tremors and paralysis got much worse with falls as days went by. I tried to grab him to stop some falls before he hit the floor and wall to hurt him worse. Mr. Harris had many things or worse medical conditions after COVID-19 vaccine. I asked physicians for medication to give him a taste for food. I cooked different type of meals and bought some to give him a smell, taste, to prevent him from being hungry. Mr. Harris whole body started tremors, going paralyze and falling more after he took COVID-19 vaccine fifth shot. I tried to catch him to prevent some falls. He had problem keeping food on his spoon or fork to eat. Mr. Harris head and right shoulder hit the floor and wall very hard, shower wall, step in shower chair. Fatigue, muscle aches, and weakness. I had to help him up from the floor which put a strain on me. He was going to PT faithfully before getting vaccines shot. He barely consumed anything, he stayed in bed feeling bad. The tremors and paralysis got much worse with falls as days went by. I tried to grab him to stop some falls before he hit the floor and wall to hurt him worse.
11/09/2023: His whole body had large tremors when sitting and walking, laying down. His body had big tremors while sitting at the table.
11-21-2023: Mr. Harris presented for a telehealth visit with his PCP and reported that he was started on steroids by his pulmonologist for SOB and falling more and his shoulder was painful. Palpitations, chest pain, vomiting, and dizziness. His weight was 155 lbs.
11-24-2023 – 01-04-2024: Mr. Harris had very severe tremors and paralyzed. He tremors for over 30 minutes. When he fell, the back of his head hit the corner of television stand. When I got to him his eyes were rolling toward his forehead, he passed out. I called 911 and told the emergency team the critical situation that was happening. I heard him calling me. I hung up the phone after I spoke with PT who was 10 minutes away from our house. The emergency team called me back. I explained to her that I had help on the way, I did not need them. I pulled him away from the television. PT got him off the floor, he tremors for over 30 minutes. I called MD, he told me to take him to the emergency room to see if there is a bleeder.
11-24- 2023: PT helped us to the car. I rushed into the emergency room and asked for a wheel chair. He really needed a stretcher because he was fallings. PT rushed into the house after I had pulled him from the television stand. He continued to have tremors and paralyze. Mr. Harris fell out of the chair in the hospital, which he needed to be in bed. His head were hitting the floor. He went to Rehabilitation but he could not stand straight up his BP was dropping. He had to slide to a wheel chair for therapy. Mr. barely ate anything due to illness from COVID-19 vaccine, which are symptoms of underlying medical conditions from COVID-19 vaccine side effects.
He lost his smell, taste, and appetite. He barely drunk fluid or health shakes. I cooked and bought different foods to help him eat. He lost weight, which lead to malnutrition. His hands and arms would not keep food on fork or spoon because of tremors and paralysis. I tried to feed him and help him stay hydrated. He got where he could not talk, he whispered. I had to hold my head down real low to hear or understand him. He was deteriorating fast. The COVID-19 vaccine took him fast. After Mr. had COVID-19 vaccine he started having some new problems and old got worse.
Mr. had many things or worse medical conditions after COVID-19 vaccine. I asked physicians for medication to give him a taste for food. I cooked different type of meals and bought some to give him a smell, taste, to prevent him from being hungry. Mr. whole body started tremors, going paralyze and falling more after he took COVID-19 vaccine fifth shot. I tried to catch him to prevent some falls. He had problem keeping food on his spoon or fork to eat. Mr. head and right shoulder hit the floor and wall very hard, shower wall, step in shower chair. Fatigue, muscle aches, and weakness. I had to help him up from the floor which put a strain on me. He was going to PT faithfully before getting vaccines shot.
January-February-March -2022: Mr. was admitted in Saint Francis Hospital for Intensive Care Unit for collapse lung. He remained in the hospital for a long time. Mr. Harris was admitted in the hospital for collapse left lung. Physician gave him Talc Power, which cause very severe pain. Staff went to his room to find out why he was hollowing so loud. It was very painful. Physician finally decided to perform lung surgery to fix the problem after having it for a long time. Mr. was put on a ventilator to help him breathe easy. He had been having left lung collapse and going to the emergency room. Doctor insert chest tube and performed surgery, which did not fix the problem. Pleurodesis was formed to prevent future collapse. It involves surgically irritating the chest wall to cause the lung and the chest wall to stick together. The doctor performed surgery for a collapsed left lung (pneumothorax) from stomach to back with long incision like open surgery (thoracotomy). The procedure include removing blebs (small air sacs) that cause the collapse, and re-inflating the lung. A chest tube was typically inserted after surgery to help drain fluid and air, and recovery involves breathing exercises and limiting strenuous activity. I asked the doctor could my husband survive without emergency treatment for collapse lung; he told me that he would die. His left lung was collapsing and physicians performed emergency surgery for chest tube to inflate lung. I got him to the hospital in time his life was in my hands. I was the caregiver for incision and cleaning it up. Breathing was taught to exercises to help clear his lungs and prevent infection. He had home rehabilitation for occupational and physical therapy.

11-16-2022: Mr. had started fallen with going paralyzed and tremors. I started hearing the loud bang on the kitchen floor. I rushed into the kitchen; he was laying on the kitchen floor. I had to help him up. He hurt his right shoulder and head.
12-3-2022 – 12- 31-2022: I called for fireman to help him off the floor. Next, I had to call for paramedic to transport him to the emergency room. He had been nausea and vomiting for four days. Mr. was admitted to Saint Francis Hospital for falls with head trauma and paralysis without loss of consciousness. He had multiple falls caused by an interaction of risk factors with paralysis and tremors. Mr. condition had been under control with a history of respiratory problem related to pulmonary fibrosis and used supplemental oxygen. Request for Benefit Form my husband had never had these many things wrong with him in his lifetime. I allege the following: colitis, diverticulitis, vomiting, weight loss, difficulty swallowing food and beverages, felt bad, left side pain, viral infection, stomach pain, low oxygen, heart fluttering, fast heartbeat, headaches, lung infections, chest and arm pain, back pain, ear/shoulder pain, sore throat, transfusion, and dehydration. Infection from viruses or bacteria. The ER physician discussed possible etiologies of electrolyte imbalance and his multiple conditions, same level falls from bed accidental hit head. Mr. was admitted to rehab on the special floor with PT and Occupational therapy. He continued PT and occupational therapy with home health.

11-29-2022- 12-03-2022: Mr. went paralyzed more often with falls. Tremors shook whole body without control of falling. On December 3, 2022, I called for fire department to help him off the floor, he was paralyzed. I called for an ambulance the second time to take him to the emergency room, he was paralyzed. He could not move waist down and arms and hands, the paralysis and tremors would go and come. He was nausea and vomiting from Friday through Monday. He had 3 infections and dehydrated because he could not drink fluid or hold it down. The doctor admitted him in the hospital on December 3, 2022 and discharged him on December 31, 2022 from physical therapy to home. I told doctors that the Covid 19 vaccine did that to him. He said he could say a lot of things about Covid 19 Vaccine, which was not good. It caused headaches from hitting his head and left shoulder on the hard floor. He complained about chills, muscle aches, decreased appetite, and taste. I knew those falls were shaking his brain up.

11-27-2023: after hitting his head on the corner part of television stand caused by bad tremors making him fall backward. Doctor recommended him going to the emergency room, he passed out, tremor lasted over 30 minutes. He wanted him checked out for bleeder. Physical therapy helped me get him off the floor to check eye movements. As a general matter, injuries warranted hospitalization, loss of function are considered serious injuries. My husband injuries warranted hospitalization numerous of time, admission critical condition, and emergency room.

12-03-2022 - 12-31-2022: Mr. went paralyzed more often with falls and tremors. I grab him to keep tremor to break falls. Tremors shook whole body without control and cause him to fall. I called for fire department the first time to help Mr. off the floor, he was paralyzed. I called for an ambulance the second time to take him to the emergency room, he tremors. Mr. could not move his body and arms because hands and the paralysis would go and come. He was nausea and vomited from Friday through Monday. He had 3 infections and dehydrated because he could not drink fluid. The doctor admitted him in the hospital and discharged him to physical therapy to home. He had gotten a little of his strength back. He had home therapy. The hospital doctor said he could say a lot of things about the Covid -19 vaccine, which wasn’t good. Mr. got headaches from hitting his head and right shoulder on the hard floor and wall. He took a lot of Tylenols for pain and headaches at home and hospital.
07-21-2023 -07-28-2023: I took my husband to the emergency room because of weakness, urgent care, mild respiratory distress. Doctor started antibiotic for infection, not getting enough oxygen. Mr. had recent emergency visit, hospitalization and coughing. He had sepsis, viral syndrome, and pleural effusion. Excess fluid between lung and chest wall due to left lung collapsed. He was in mild respiratory distress and leukocytosis is a condition characterized by abnormally high number of white blood cells in his bloodstream. It is a crucial role in the body's immune system, fighting infections and inflammation. He had chest and side pain; he needed medical treatment right away. Trouble sleeping, fluttering in chest, muscle weakness, and dizzy.
10-06-2023: Mr. presented to his PCP to discuss his laboratory results and multiple medications. He reported feeling fair overall. He had nosebleeds, chest pain, palpitations, SOB, vomiting, dizziness, or headaches. His PCP said his diabetes was healed because Mr. loss weight. I called the neurology, but he could not perform CT with dye with him being in the hospital. His weight continues to drop. He loss 90 pound over 2 years due to poor eating habit. He had numerous surgeries to expand left lung. I witness everything that happen to Mr., I was his caregiver. His serious injuries required hospitalizations or resulted in a significant loss of function or disability, and covers both his physical complications and injuries. His tremors and paralysis got much worse and barely could move arms with excessive falls. He could not move his legs or body to turn over in bed when he arrived to the nursing home rehabilitation. He just barely moved an arm. Mr. never seen all the different physicians prior to vaccines. Before he went to the hospital, he walked some and got in a chair. He stopped moving all parts of his body. I could not take care of my husband at home alone. I had called hospice to see him at home. I had to do more since he could not do anything alone.

• 11-07-2023: Chest pain, dyspnea, ankle edema, fatigue, felt weight on his chest, fall constantly. Left neck pain travel down left arm attributed to cervical disease, no syncope.

• 11-09-2023: Mr. whole body had large tremors when sitting and walking, laying down. His body had big tremors while sitting at the table. After 4 to 5 days, I noticed his whole body had tremors and falls hitting the floor and wall. I tried to catch him to break fall before he hit the floor.

• 11-16-2023: Mr. presented to the neurologist for follow-up. Mr. Harris wife asked if Mr. Harris’ worsening weakness, balance, and tremors related to his COVID-19 vaccine. The neurologist recommended continued follow-up with his cardiologist and pulmonologist as he felt his underlying medical issues contributed to weakness and unsteadiness overall poor function.

• 11-21-2023: Mr. presented for a telehealth visit with his PCP and reported that he was started on steroids by his pulmonologist for SOB and falling more and his shoulder was painful. He had palpitations, chest pain, vomiting, and dizziness. His weight was 155 lbs.

Hospital employees would not let me into the emergency room so I went home. The nurse kept calling me asking questions. He went to Saint Francis Hospital. Mr. fell from a chair in the hospital and hit his head on the hard floor. He needed to be in bed. He needed test done with Dye for brain. But dye could not be used on him. Saint Francis Hospital do not perform test with Dye.

He had low magnesium and potassium. Mr. was paralyzed before he went from Saint Francis Hospital specialized floor rehabilitation. He could not barely move arms to eat. 01-04-2024: He transferred from Saint Francis Hospital and his right foot was hurting bad to skilled nursing home for rehabilitation. Physician check him out and order a cream for his foot.

The Countermeasures Injury Compensation Program (CICP) provides compensation for covered serious injuries or deaths based on compelling, reliable, valid, and medical caused by the administration of vaccine. The administration or use of COVID-19 vaccine the covered countermeasure and onset of the injury occurred after the administration or use to prove that an injury is the direct result of a covered countermeasure. COVID-19 vaccine made my husband sick enough to seek emergency care. The acute respiratory illness categorizes the percentage of emergency department (ED) visits and the extent of respiratory illness.
He had acute respiratory illnesses and respiratory infections COVID-19 virus associated with respiratory illness. The symptom chills, cough, shortness of breath or difficulty breathing, sore throat, runny nose, loss of taste and smell, fatigue, muscle aches, headaches, and nausea or vomiting. Persistent pain or heavy pressure in his chest, COVID-19 caused infections. Injuries that warranted hospitalization and led to significant loss of function or disability.
It provides compensation for covered serious injuries or deaths based on compelling, reliable, valid, medical caused by the administration or use of a covered countermeasure. Temporal association between the administration or use of the covered countermeasure and onset of the injury, it was a certain time after the administration or use of COVID-19 vaccine. An injury was the direct result of a covered countermeasure. The Countermeasures Injury Compensation Program (CICP) provides compensation for covered serious injuries or deaths based on reliable, valid, medical evidence, directly caused by the administration or use of COVID-19 vaccine. I applied for the Countermeasure Injury Compensation Program on January 3, 2023. They sent me a letter that attorney can help me with claim for husband.

Injuries, disabilities, illnesses, and deaths occurred after vaccine administration. Illness, injury or condition covered shoulder injury related to vaccine administration and falls.
Manifests as shoulder pain and limited range of motion occurring after the administration of a vaccine in the upper arm.

Mr. had COVID-19 vaccine with symptoms ranging from mild symptoms to severe illness. Symptoms may appear 5-14 days after vaccine which started mild, and progressed to more severe symptoms. Risk factors for severe illness from COVID-19 vaccine made him very sick. This includes having a weakened immune system, certain disabilities or underlying health conditions, COVID-19 risk factor and risks that might affect him.

These serious injuries, paralysis and tremors contributive to my husband death. It killed him slammed his head against hard floors and wall; death was caused by administration and used of COVID-19 vaccine. His head was slammed against hard floors and walls; death was directly caused by COVID-19 vaccine. It caused significant loss of function and hospitalizations.
I ask that the right thing be done because COVID-19 vaccine contributive to my husband death with serious injuries, body harm, paralyzed him. It gave him bad tremors, low blood pressure, which damages by hitting his head frequent on floors, wall with many falls. Mr. Harris had some medical problems before COVID-19 vaccine. Behind the vaccine he got new or more problems or concussions. The COVID-19 vaccine, which is traumatic brain injury that affects function.
Mr. had many things or worse medical conditions after COVID-19 vaccine. I asked physicians for medication to give him a taste for food. I cooked different type of meals and bought some to give him a smell, taste, to prevent him from being hungry. Mr. whole body started tremors, going paralyze and falling more after he took COVID-19 vaccine fifth shot. I tried to catch him to prevent some falls. He had problem keeping food on his spoon or fork to eat. Mr. head and right shoulder hit the floor and wall very hard. Fatigue, muscle aches, and weakness. He was going to PT faithfully before getting COVID-19 vaccine shot.
11/09/2022: about 4 or 5 days after he got 2nd Booster vaccine, Mr. almost became bed ridden eating, and vomiting. He slid off the bed to the floor because he was paralyzed. His head hit the night stand and burst left ear; he bled a lot. I finally knew as I observed him slide off the bed, his body was paralyzed. He could not move arms to catch walker to pull himself up off the floor. He used a walker but did not prevent him from hitting things and falling. I begged him to eat and rushed to grab him to prevent hitting his head on the floor, walls, bathroom and falling. He always had 2 or 3 infections every time he went to the hospital for emergency treatments. After COVID-19 vaccine, he had never had so many problems and complications before in his life. I told the doctor that he had COVID-19 vaccine. He told me there were a lot of things he could say about COVID-19 vaccine. Before Booster vaccine he was going to PT and doing fair, looked forward to PT.
12/01/2022 I called the fireman twice to help Mr. because he became paralyzed and had tremors falling. He could not sit down; he fell off the couch and bed due to being paralyzed and bad tremors.
• 01/10/2023 and 01/27/2023: Mr. presented to his PCP for post hospital follow-up and a routine annual examination. He had chest pain, palpitations, dizziness, or headaches. He often had nosebleeds. The blood pressure readings were normal at both visits. The PCP noted his weight was 205 pounds (lbs.), he had lost 17 lbs. during his hospitalization, and his body mass index (BMI) was in the overweight range. His Losartan was held due to low blood pressure.

• 05/09/2023: Mr. presented for telehealth follow-up visit with his PCP and reported he was told by his orthopedist that he had arthritis. He had recently fallen which made his knee pain worse and shoulders. He also reported he had poor appetite and was not eating much. He had chest pain, palpitations, SOB, fevers, dizziness, weakness, or vomiting. He had a six-pound weight loss from his previous visit. He had headaches frequently I had to buy Tylenol after COVID-19 vaccines for his headaches.

• 06/02/2023: Mr. had a transthoracic echocardiogram that showed mild right ventricular enlargement with borderline impaired systolic function and a normal left ventricular ejection fraction.

• 06/03/2023: Mr. completed a nuclear stress test that showed normal left ventricular volumes, normal wall motion and thickening, and a normal L VEF. The electrocardiogram was negative for ischemia with occasional premature atrial contractions (PACs).
• 06-12-2023 – 06-13-2023: Mr. seek emergency urgent care. Severe dehydration is an emergency and needs treatment in a hospital. Loss of muscle strength because of inactivity and insomnia. Mild or moderate dehydration could be treated at home. Drink an oral rehydration solution to restore proper number of fluids and the blood. He received treatment with fluids. Weakness, infections, imbalance in mineral levels, low potassium, dizziness, no sweat, and chills.

• 06/22/2023: Mr. presented for a telehealth visit with his PCP and reported that he was evaluated in the ED for supraventricular tachycardia due to dehydration. His heart rate (HR) returned to normal without any changes to his medications. He had palpitations, chest pain, SOB, fatigue, vomiting, and fever. His vital signs were significant for and elevated BP and a nine-pounds weight loss from the previous visit. The diagnosis was supraventricular tachycardia.
11/24/2023: I heard Mr. calling me, he hit his head on the television stand, fell backward and passed out with eyes rolling up toward his forehead. I could only see the white part of his eyes. Hypoxemic respiratory failure. Hospital employees would not allow me into the emergency room due to COVID virus. The nurse kept calling me asking questions. Mr. Harris fell from a chair in the hospital and head hit the floor hard. He needed to be in bed. CT of brain was normal, mild diffuse cortical. He needed test done with Dye for brain. Saint Francis Hospital do not perform test with Dye. I got him to the hospital in time his life was in my hands.
• 07/03/2023: Mr. presented for a telehealth visit with his PCP and reported a poor appetite. he denied diarrhea, vomiting, chest pain, palpitations, dizziness, headaches, fever, or nosebleeds. His vital signs were significant for and elevated BP.

• 07/04/2023: I took Mr. to Saint Francis Hospital for emergency treatments. He had stomach pain, weakness, vomiting and an infection. He was treated with IV Fluid and given medications for prescription for home and discharged.

07-21-2023 -07-28-2023: I took my husband to the emergency room because of weakness, urgent care, mild respiratory distress. Doctor started antibiotic for infection, not getting enough oxygen. Had recent emergency visit, hospitalization and coughing. He had sepsis, viral syndrome, and pleural effusion. He was in mild respiratory distress and leukocytosis is a condition characterized by abnormally high number of white blood cells in his bloodstream. He had chest and side pain, he needed emergency treatment right away, pleural effusion could be a sign of serious underlying condition. Trouble sleeping, fluttering in chest, tingling, feeling light headed.
• 08-01-2023: Mr. presented for a telehealth post hospitalization visit with his PCP
and reported he was given “fluid pills” for fluids over his lungs and heart with weakness, and he was having “cramps” over his abdomen wall. He had chest pain palpitations, SOB, vomiting, dizziness, and headaches.

• 08-15-2023: Mr. presented for telehealth visit with his PCP and reported he had seen his orthopedist and felt very tired, lightheaded when he stood up, and that he was not drinking enough fluids. He had palpitations, chest pain, SOB, vomiting, or headaches. The PCP noted that Mr. had an history of atrial fibrillation, and she opined that his high HR was likely dehydration and recommended hydration.

• 09-20-2023: Mr. presented for a telehealth visit with his PCP and reported that he was having bad tremors. He had chest pain, palpitations, dizziness, headaches, SOB, fever, or nosebleeds.
09-27-2023: Mr. presented to a scheduled appointment with neurology. I looked for answers that would help him find diagnose of his brain with dye. He only seen the doctor about twice or three times. He was not drinking enough fluid. Doctor could not perform test of head with dye. I just asked the doctor if he could have “potential parkinsonian features on exam”. His right hand had tremors which COVID-19 vaccines could have caused that problem. He didn’t know without performing test with dye. Occasion he had very little tremors with hand on right side of body for a few years, weakness, and unsteadiness on feet. It had been all over his body after being on COVID-19 vaccine. I explained to the doctor what was happening to him. He needed a wheel chair to go for appointments due to paralysis and tremors. He almost fell out of the wheel chair.
• 10-06-2023: Mr. presented to his PCP to discuss his laboratory results and multiple medications. He reported feeling well overall. He had fever, nosebleeds, chest pain, palpitations, SOB, vomiting, dizziness, or headaches.
His primary care doctor said his diabetes was healed because Mr. Harris loss weight. I called the neurology, but he could not performed dye tests with him being in the hospital and medical conditions. His weight continues to drop. He loss 90 pound over 2 years due to poor eating habit. He had numerous surgeries to expand left lung. I witness everything that happen to Mr. Harris, I was his caregiver. His serious injuries required hospitalizations or resulted in a significant loss of function or disability, with a detailed explanation that covers both his physical complications and injuries. The established CICP is to provide compensation to eligible individuals for serious physical injuries or death directly caused by the administration. COVID-19 vaccine paralyzed my husband and gave him permanent paralysis and tremors for life-time. Mr. conditions deteriorated and he kept headaches. In January 2024, I had told the nurse about Mr. Harris before she called me, he had deteriorated, which I already knew. He could not move his legs or body to turn over in bed when he arrived to Nursing Home Rehabilitation and before he was discharged from Saint Francis Hospital. He just barely moved an arm. He had never experienced these types of episodes prior to COVID-19 vaccines. Mr. never seen all the different physicians prior to COVID-19 vaccines. Before he went to the hospital, he walked some but there were times he became paralyzed, tremors and kept falling. He stopped moving all parts of his body. I could not take care of my husband at home alone. I had called hospice to see him at home. I had to do more since he could not do anything alone.
• 11-07-2023: Chest pain, dyspnea, ankle edema, fatigue, weight on chest, fall constantly. Right Left neck pain travel down left arm attributed to cervical disease, no syncope.

• 11-16-2023: Mr. presented to the neurologist for follow-up. Mr. wife asked if Mr. worsening weakness, balance, and tremors were related to his COVID-19 vaccine. The neurologist recommended continued follow-up with his cardiologist and pulmonologist as he felt his underlying medical issues contributed to weakness and unsteadiness overall poor function.

• 11-21-2023: He presented for a telehealth visit with his PCP and reported that he was started on steroids by his pulmonologist for SOB and falling more and his shoulder was painful. He had palpitations, chest pain, vomiting, and dizziness. His weight was 172 lbs.
11-24-2023 – 01-04-2024: Mr. was having very severe tremors and paralyzed. He tremors for over 30 minutes. When he fell, the back of his head hit the corner of television stand. When I got to him his eyes were rolling toward his forehead, he passed out. I called 911 and told the emergency team the critical situation that was happening. I heard him calling me. I hung up the phone after I spoke with PT who was 10 minutes away from our house. The emergency team called me back. I explained to her that I had help on the way, I did not need them. I pulled him away from the television. PT got him off the floor, he tremors for over 30 minutes. I called MD, he told me to take him to the emergency room to see if there is a bleeder.
12-03-2023 - 01-04-2024: He was admitted to Saint Francis Hospital numerous of times. He fell out of his chair many times. It turned over in the hospital with paralysis. His head hit the floor hard. He had no quality of life, could not visit family. I knew when he stopped talking, he did not have long to live. He could only whisper. He could not eat. The last thing I said to him on 01-22- 2024 is I love you; he told me that he loves me. I did not know that would be the last time. This robbed him of his livelihood. Mr. Harris thought that if he got COVID-19 vaccine it would prevent him from getting Covid. Instead, he got something much worse that killed him and took him away from me. We were married for 33 years.

01-04-2024 - 01-23-2024: Spouse was admitted to Saint Francis Hospital numerous of times. He fell out of his chair many times. It turned over in the hospital with paralysis. His head hit the floor hard. He died on 01- 23-2024: He had no quality of life, could not visit family. I knew when he stopped talking, he did not have long to live. He could only whisper. The last thing I said to him on 01-22- 2024 is I love you; he told me that he loves me. I did not know that would be the last time. This robbed him of his livelihood. He thought that if he got COVID-19 vaccine it would prevent him from getting Covid. Instead, he got something much worse that killed him and took him away from me. We were married for 33 years. I called family members to go see him because he did not have long to live.

PT helped us to the car the day after Thanksgiving. I rushed into the emergency room and asked for a wheel chair. He really needed a stretcher because he was fallings. PT rushed into the house after I had pulled him from the television stand. He continued to have tremors and paralyze. Mr. fell out of the chair in the hospital, which he needed to be in bed. His head hit the floor. He went to Rehabilitation but he could not stand straight up his BP was dropping. They gave medication for his PT. He had to slide to a wheel chair for therapy on a board.
He lost his smell, taste, and appetite. He barely drunk fluid or health shakes. I cooked and bought different foods to help him eat. He lost weight, which lead to malnutrition. His hands and arms would not keep food on fork or spoon because of tremors and paralysis. I tried to feed him and help him stay hydrated. I had to hold my head down real low to hear or understand him. He was deteriorating fast. The COVID-19 vaccine took him fast. After Mr. had COVID-19 vaccine he started having some new problems and old got worse.
01-04-2024 - 01/23-2024: I called the nurse at a skilled nursing facility for rehabilitation. I told her that my husband was deteriorating fast. She told me that he was not deteriorating. The following day the nurse called me back, she told me he was deteriorated. I knew more than her. I told family members that need to see him go because he only had a short time before he is gone. There was no report of headaches, chills, frequent infections, lung collapse, paralysis in the submitted medical records. The submitted medical records indicate that Mr. cause of death was illness and injuries.
Spouse died on January 23, 2024, because of his injuries resulting from the use of a COVID-19 vaccine. He stopped walking or talking, bedridden, he was supposed to came home on January 24, 2024 with Hospice o help take care of him.
