What was your life like before you received the COVID-19 vaccine?

Normal, zero issues

Describe the symptoms and the timeline of the reaction.

6/5 ended up in ER with multiple near syncope episodes, mild chest pain, and surging sensation to extremities. Was stabilized and discharged after clear x-ray and ct with contrast. on 6/2 noticed prior to the event noticed chest pain and extreme dizziness in the pm. 6 weeks prior to event off and on shortness of breath. Really usual for someone who exercises 6-7 times a week and is a former athlete. Since the event 6/5, have had 2 episodes near syncope usually after caffeine. Daily dizziness that was never present pre-event.

Describe the solutions that helped your symptoms

Avoiding caffeine (something ive had consistently for 10 years+ with no issues). Over hydrating seems to help things calm down. Still searching for solution to daily dizziness.
