What was your life like before you received the COVID-19 vaccine?
I considered myself to be of good health. Being semi-retired working part time. Had energy and strength to do small jobs around the house. Hobbies of fishing were enjoyable.
Describe the symptoms and the timeline of the reaction.
Beginning in Nov., '21, I began having generalized weakness and fatigue. Also shortness of breath on exertion. I have visited my primary care physician throughout this past year, several tests have been done, but no specific cause found. Test included Pulmonary function test, extensive blood panel, cardio. stress test, CT of chest(high resolution) and cardio cath. done Sept. 23, '22 after which on the 30th diagnosed with blood clot in right forearm (site of procedure). Pending ultrasound exam of lower extremities scheduled for 12 Dec., '22
