Describe the symptoms and the timeline of the reaction.

"7/19/21

I am a 52 yo old female with pre-vaccine diagnosis of spondyloarthropathy, fibromyalgia, migraine and Hashimoto’s disease. All of these conditions were stable /under control with historic methotrexate & biologic injections, steroid taper, gabapentin, and levothyroxine.

After 2nd dose Moderna vaccine (May 6, 2021), I experienced a ‘typical’ response for several days of fatigue, nausea, headache and body aches. After this resolved, I had a severe arthritis flare necessitating the use of a cane. The arthritis flare lasted 5-7 days.

Approximately 3-4 weeks post 2nd dose I started experiencing the following:

* extreme debilitating fatigue

* heart palpitations

* brain fog/short term memory loss

* shortness of breath with any activity

* dizziness

* loss of appetite & unintended weight loss

* chronic insomnia

* myalgia in neck & back

* sharp increases & decreases in blood pressure & hr

* swollen lymph nodes

* sore throat

* metallic taste in mouth

* tremors/ shaking in arms & hands

* pale appearance

So far blood labs for thyroid and anemia are normal. CBC and chem panel also normal. Will be having inflammatory markers checked. Doctor ordered a stress echocardiogram, Holter heart monitor, brain MRI, and chest CT.
