To whom this may concern,

My name is C. I am patient number # in Moderna's

covid-19 "" vaccine "" trial #. I received my first dose August 2020 and the second dose September 28th, 2020.

After the first dose , arm pain never resolved.

After the second dose, I had head twitching, stuttering, brain fog, loss for words , off balance, confusion, forgetfulness, head pressure, low grade fever, unwell feeling, that lasted 2 weeks.

After the neurological adverse symptoms began to wear off, I became more aware of how bad my arm/ shoulder was getting from the injections.

Long story short, I ended up having to have shoulder surgery.

It was determined by orthopedic surgeon that the injury was from adverse inflammation reaction from the Moderna vaccine but not caused by improper needle placement.

I may have nerve damage. To this day, I am still in physical therapy. I am still in pain . My quality of life is not what it was before Moderna's shots.

I have had the left axillary lymph nodes removed because they were enlarged still at 8 months post vaccination.
